How readers, providers, regulators, and authorized representatives can report a factual directory error with useful evidence and without sensitive data.
Report a factual issue
Identify the exact listing URL and field, explain what appears wrong, provide the correct value, and cite a current public source from the responsible regulator, authority, or provider. Include the date you checked it. Do not send resident records, staff files, identity documents, account information, or other confidential material.
Review process
The administrator may verify the evidence independently, contact the organization through an official channel, amend the field, add context, or leave it unpublished while facts remain uncertain. Requests to remove accurate public-interest information are evaluated under applicable law and the editorial policy, not accepted automatically.
Claims and urgent concerns
Authorized representatives can use the separate listing-claim form. A claim does not bypass source review. This directory is not an emergency, complaints, safeguarding, inspection, or clinical service; urgent risks and formal complaints should go directly to the appropriate provider, regulator, authority, ombudsman, safeguarding body, or emergency service.
